PROBLEM
Service user found that getting back in the community is a big step forward. They said people are vulnerable, and they might not know what to do in the community or what help is actually available. Service users want have to develop the skills, knowledge and resources to avoid people being re-called to Hospital.
PROPOSAL
Identified a need for more focus on discharge pathways collaborating and pulling together all the available information.
Sharing lived experiences of admission, discharge and recall to hospital.
Identified as similar themes emerging from the ‘stuck patient groups’

The Idea came from a service user who was recalled earlier in the year and wanted to share his experience, he was then supported by another peer and ward staff to capture his ideas for a group/project that would focus on the process of moving on from hospital, having a forum to share experiences and better understand the process, managing potential anxieties and what support is available, sharing lived experiences.
Initially called “Hostel survival” and now “the moving forward” project service users want to create a course of sessions that run focusing on how to make the most out of time in hospital focusing on rehab and positive impacts such as therapeutic engagements, sports & activity, increased family & carer involvement along side preparation for DX to community with practical support such as learning to manage finances, community teams sessions, lived experiences of peers, vocational opportunities. Link in with different teams involved with care along the process and after discharge to ease anxiety and better knowledge of process.

Next Steps – when moving on
